Separate the tooth from the middle (buccolingualty & mesiodestally) to remove the affected part and preserve the rest of the tooth this procedure called:
A- Hemisection ***
B- Bisection.
Hemisection is a surgical procedure usually performed on the lower molars to maintain a root. The roots are separated by means of a strawberry.
A root is then avulsed. After severing the pulp cavity to separate the roots, the remaining root should be provided with root restoration and possibly coronal restoration with anchorage.
In case of subsequent prosthetic treatment, the root still present serves as a bridge abutment.
Other indications for a hemisection:
- extensive subgingival root decay;
- deep periodontitis on a single root;
- dental fractures, when one of the roots is worth preserving.
This surgical technique consists in separating the roots of a tooth, usually a lower molar, in order to favor the use of hygiene instruments (interdental brush) between the roots of this tooth, before crowning it.
This technique, unreliable in the long term, has fallen into disuse since the advent of modern implantology. However, it can allow to wait before moving to the implant.
